Serve
provides support to people in their own homes and relief and
assistance to carers.
Referrals
will be accepted from any source providing that the recipient is
aware of and has given their approval to the
referral.
Before
any service is offered we will carry out an assessment which is free
of charge to enable us to ensure that the right carer is placed with
the client. All our carers are employed by us, are fully trained and
regularly monitored.
Coping
with everyday tasks can sometimes be difficult after a stay in
hospital. Serve aims to provide practical help, support
and advice.
Hospital Aftercare
This
service is available to older people who have been discharged
from hospital.
We
understand that coping with everyday tasks can be difficult when you
have been in hospital. It is even worse if you are living alone or
have no one to take care of you.
Hospital
aftercare can be essential to enable a discharged patient time to
re-adjust and become independent once more. We can provide transport
if you need to go shopping or if you have a medical appointment. We
can also help you with advice on benefits and
entitlements.
Our
carers hold a food hygiene certificate and are available to prepare
a hot cooked lunch or light snack.
With
you permission we will ensure that appropriate contact is made with
other relevant community services, for example your GP.
Carers
Relief
If
you know that you are going into hospital or about to be discharged,
then contact us. We will
endeavour to visit you within 48 hours, either in hospital or at
home, to discuss what help you might need.
We
can offer a sitting service for your loved ones to give you a break
on a regular basis or as a one-off. We can provide a caring
companion who can chat, read, listen or make a cup of
tea.
If
you are going a holiday we can arrange a regular monitoring service
to give you peace of mind.
Personal
care is not provided but we can signpost you to a recommended agency
or organise it on your behalf.
Because
Serve is a registered charity and these services are not directly
funded, a charge has to be made to offset our
expenses.
Services
We
can offer a whole range of support services to help you cope. This
can be in the form of practical help and advice with the aim of
helping you to re-adjust and become independent
again.

can assess your home to see if you are able to get up and down
stairs or in and out of the bath.

We
can carry out minor works to install grab rails and other
aids.

We
can arrange for you to hire a whole range of mobility equipment
like wheelchairs, zimmers and commodes.

can arrange to take you shopping or to the chemist or to go and do
it for you.

We
can arrange transport to the Doctors or to hospital for check
ups.

We
can come and visit you at home and help with form filling and
benefits advice.

We
can help with domestic chores and preparing a
meal.

We
can offer you companionship and friendship in one of our Day Care
Centres where you will be able to have a meal and help with
bathing.
There
are a range of nominal charges for the various services. Initial
assessments and advice are provided free of charge.
